Key Points Restoration of TIMI 3 flow after treating stent thrombosis reduces repeat stent thrombosis but not death or MI in the following year. Understanding the mechanisms differentiating early, late and very late stent thrombosis will direct better initial treatment strategies. Angiographic markers of coronary physiology (TIMI, TPG, Collateral grade) do not substitute for measured more quantitative indices of coronary blood flow.
