We report a novel approach in which successful unjailing of the septal perforator was performed through the side branch of a coronary stent using a rotational atherectomy device for the purpose of alcohol septal ablation in a patient with symptomatic hypertrophic obstructive cardiomyopathy. © 2014 Wiley Periodicals, Inc.
