Untreated, ruptured vertebral dissecting aneurysms are associated with high incidences of rebleeding and consequent mortality. Double‐origin of the posterior inferior cerebellar artery (PICA) is an infrequent congenital variation, which showing predilection for the formation of intracranial aneurysm. We describe an endovascular approach toward the treatment of a ruptured vertebral dissecting aneurysm associated with double‐origin PICA. © 2010 Wiley‐Liss, Inc.
