This report describes a retrograde wiring technique, using intravascular ultrasound, for a blunt chronic total occlusion with a side branch at the site of occlusion of which the operator has difficulty of awareness of the proper re‐entry point with the retrograde wire angiographically. © 2009 Wiley‐Liss, Inc.
