Paget‐Schroëtter syndrome, or primary thrombotic occlusion of the axillary‐subclavian vein, is diagnosed in a 42‐year‐old man subsequently treated with thrombolysis and anticoagulation. Treatment strategies, including early and delayed surgical options, are discussed to determine the best risk/benefit ratio. Cathet Cardiovasc Intervent 2003;59:71–76. © 2003 Wiley‐Liss, Inc.
