A report is given on patient—staff collaboration in the Dr Henri van der Hoeven Clinic at Utrecht, The Netherlands, a facility for residential treatment of personality disorders. It is the hospital's conviction that, in cases of personality disorder, such collaboration must be intensive and extensive. The conviction rests on theoretical premises that have continued to serve as guidelines for everyday treatment operations for 36 years. After a brief description of the clinic and its patients, the underlying premises are sketched. The main focus of the article, however, lies in description of how these premises can be empirically expressed, their effects and side‐effects, including problems and failures experienced by patients and staff.
