Introduction The oral cavity is the most common area of mucosal involvement in pemphigus vulgaris (PV). Rituximab has been used in the treatment of severe and extensive PV. PV limited just to the oral cavity can be an initial manifestation of severe disease. This is a retrospective study of four PV patients who had treatment resistant oral PV and were treated with rituximab (lymphoma protocol). Case Presentation All four patients were young females who failed to respond to multiple oral conventional treatments for at least 12 months. All four patients had a complete response to rituximab. Two patients were able to discontinue systemic steroids. Conclusion Rituximab can be used as an effective rescue agent in the treatment of recalcitrant oral PV.
