A patient developed a syndrome of head tilt, limited upgaze, and convergence‐retractory nystagmus. Pathological examination revealed a single 0.6 cm diameter metastatic tumor nodule in the right tegmentum of the midbrain extending from the posterior commissure to a plane through the lower end of the lateral geniculate body. The unique features are the lesion's small size and the fact that it was unilateral.
