A 68‐year‐old man withpolycythemia rubra vera developed gradually progressive paraparesis followed by an acute, painless paraplegia. Extradural thoracic spinal cord compression was found to be secondary to extramedullary hematopoiesis (EH). The neurological complications of EH are reviewed.
