Supranuclear paralysis of gaze was a prominent manifestation of Whipple's disease in 2 patients with dementia, 1 of whom also had bilateral uveitis. Because of the absence of gastrointestinal problems in these patients, correct diagnosis was delayed. Whipple's disease is a treatable disorder and should be considered in patients with otherwise unexplained supranuclear gaze paralysis and uveitis.
