We report a patient with acute large granular lymphocyte (LGL) leukemia, presenting as acute myelofibrosis (AMF). The leukemic cells were immature T‐cells (CD5+, CD7+, CD16−, CD56−, CD57−, and CD41−), had monosomy 7, and secreted large amounts of Transforming Growth Factor‐β1(TGF‐β1). The serum levels of interleukins (IL)‐2, ‐2R, ‐6 and ‐8 were elevated, while the IL‐1<β, IL‐4, and tumor necrosis factor‐α were normal. © 1995 Wiley‐Liss, Inc.
