The theoretic basis of neoadjuvant chemotherapy is to enhance locoregional control by tumor debulking or sensitizing the tumor to radiation, and to decrease distant metastasis by treatment of occult micrometastasis. Nasopharyngeal carcinoma, a very chemoresponsive tumor with a high incidence of distant metastasis, should be an excellent head and neck malignancy for neoadjuvant chemotherapy to display its potential. However, whether chemotherapy should be used routinely in the treatment of nasopharyngeal carcinoma is still controversial. See also pages 2270‐83.
