The presenting features of vaginal bleeding and abnormal cytologic smears in a elderly patient with simultaneously occurring acute myelogenous leukemia (AML), were due to massive infiltration of the uterine cervix and endometrium by immature myeloid elements or granulocytic sarcoma. The abnormal cytologic smears and uterine mass were initially believed to represent a poorly differentiated lymphocytic lymphoma. The diagnosis of granulocytic sarcoma was confirmed with the naphthol AS‐D chloroacetate stain that demonstrated activity in the cytoplasm of the neoplastic cells.
