Overt autoimmune hemolytic anemia in association with cancer other than leukemia and lymphoma is rare. This paper records the first case of Coombs' positive hemolytic anemia associated with colon cancer. A 49‐year‐old woman suffered from severe anemia and mild icterus. The prominent hemolysis was not suppressed by steroid treatment. The direct and indirect Coombs' test was positive. She died five months after the onset of her illness. No metastases were present. Reticuloendothelial hyperplasia and hemosiderosis of the bone marrow, the spleen, and the liver were marked.
