A patient with myelofibrosis and myeloid metaplasia diagnosed 11 years prior to death showed multiple lytic bone lesions with pathologic fractures for 9 years. At autopsy, a sarcomatous tumor composed of bone marrow elements, including megakaryocytes and myeloid precursors, was found in bones, lymph nodes, and liver. Transition of myelofibrotic bone marrow into myeloid sarcoma suggested that myeloid sarcoma is a malignant tumorous form of myelofibrosis and myeloid metaplasia.
