We have recently seen two patients with hepatocellular carcinoma who presented with vertebral metastases and nerve root compression. The details of their cases are reported, and the literature concerning this presentation of hepatocellular carcinoma is reviewed. It is concluded that any patient with vertebral metastatic disease of unknown origin should be investigated for possible primary liver carcinoma. The increased awareness of this presentation of hepatocellular carcinoma in Africa probably reflects the larger overall experience with the tumor in that continent.
