Urinary cytology studies were performed on 306 patients from 1963 to 1969; 245 were suitable for critical review. Accurate diagnoses were made cytologically in 80% overall. Twenty‐three per cent of all negative reports were false, and 4% of all positive reports were false. Maintenance of strict criteria of malignancy is emphasized as a means of achieving a low false‐positive rate. Examination of multiple urine samples obtained by barbotage is suggested as a means for improving the false‐negative rate. Selected patients with both false‐positive and true‐positive cytologic findings are presented in detail, including those in whom malignancy was first demonstrated by cytologic means.
