The serosal rupture due to tumor cells is one of the most important factors influencing the prognosis of gastrectomized patients with advanced gastric carcinoma infiltrating beyond the muscularis propria of the stomach. However, at present, the positive signs of this rupture by one representative tissue section is not established. We would like to present here the standard histologic criteria for this subject. Four different growth patterns of the tumor, classified here, showed different characteristics concerning the incidence rate and way of the rupture and then adjuvant changes of the serosal surface near the rupture by examining the serial sections of the involved stomach wall. Practically, our established criteria, applying to 305 cases on whom radical surgeries were performed, fit well in judging the prognosis, especially in cases with wide extent of the serosal rupture due to tumor cells.
