Cancer of the accessible primary sites can be classified quite easily by the TNM system. However, rigid adherence to this system becomes difficult and problems are encountered in classifying the extent of involvement in the less accessible sites. Recent examples of the necessity for modifying the TNM clinical classification are carcinoma of the colon and rectum where surgical findings are much more important than clinical findings only. After a study of several years, a proposed classification for carcinoma of the colon, developed at the Sixth National Cancer Conference in 1968, has been subjected to field‐trial analysis. A similar study for cancer of the rectum and anus will be initiated by the Task Force on Colon, Rectum, Anus of the American Joint Committee for Cancer Staging and End Results Reporting.
