An elderly male with acute leukemia in hematologic remission developed acute urinary retention secondary to prostatic enlargement. Surgical pathology of the prostate revealed that the syndrome was caused by leukemic infiltration. Another patient with leukemia developed a similar syndrome which was successfully relieved by low‐dose radiotherapy. Subsequent postmortem pathology did not demonstrate leukemic infiltration or benign hypertrophy. It is concluded that radiotherapy destroyed the leukemic cells relieving the patient's urinary retention. These cases illustrate the perplexing problems that can be caused by extramedullary leukemic infiltrates.
