The clinical and pathologic findings and follow‐up are reported in 7 cases of testicular plasmacytoma. The age range was 26 to 66 years. In 4 patients, the initial symptom was testicular enlargement; in the other 3, testicular enlargement followed extratesticular symptoms. In all cases, the plasma cell masses were neoplastic rather than reactive, and all tumors were interpreted as being malignant. The tumor cells varied in degree of differentiation. Some were readily identified as plasmacytic, while others more closely resembled reticulum cells. In all but one case, the testicular plasmacytoma was eventually associated with extratesticular plasma cell myeloma. Although tumor‐free intervals may follow orchiectomy, the prognosis of patients with testicular plasmacytoma is poor. In most instances, the testicular mass should be considered as part of a systemic neoplastic, myelomatous process.
