Six hundred and nineteen surgical prostate tissue specimens were analyzed for their LDH isoenzyme pattern, and the results were correlated with tissue pathology as determined by routine pathologic processing techniques. In 87 cases, 2 or more areas of each prostate were submitted to the same procedures using the ratio of LDH V to LDH I as an index of malignancy. An index value greater than one was noted in 77.6% of pathologically malignant prostates; an index value less than one was seen in 86.1% of pathologically benign glands. The LDH V/I index was elevated throughout 80% of malignant glands, even though the carcinoma occupied only one area of the prostate. The clinical value of the LDH V/I Ratio index in early diagnosis of carcinoma of the prostate is discussed.
