Abstract A 55‐year‐old man with a pharyngoesophageal diverticulum that had been, followed for 10 years developed progressive symptoms. The diverticulum was resected and was found to contain an invasive carcinoma. It would appear that there is little justification for conservative treatment of pharyngoesophageal diverticula as they may undergo malignant transformation, and resection can be performed with low morbidity and mortality.
