For the first time, 2 cases of solitary plasmacytoma of the spine were diagnosed at the time of surgery by means of rapid (40‐minute) biochemical analysis of biopsy material. Tissue extraction (by a technique previously described) successfully demonstrated a monoclonal immunoglobulin in high concentration in each lesion, providing reliable evidence for the correct diagnoses, whereas both radiologic and pathologic diagnoses proved to be incorrect in both cases. “Incipient” myelomatosis of the spine was demonstrated in a third (autopsy) case by means of extraction of uniquely disseminated miliary‐like lesions which, by direct smear and Wright's stain, showed characteristic small colonies of plasma (myeloma) cells.
