This report documents a rare case of primary adenocarcinoma (adenoacanthoma) of the rectovaginal septum arising from an area of endometriosis. It is particularly interesting that successive biopsies revealed the evolutionary development of adenocarcinoma from endometriosis through a progressive sequence of changes similar to those known to occur in the endometrium itself.
