Abstract The occurrence of hypertrophic gastritis in an 87‐year‐old woman is reported. Weakness, weight loss and epigastric fullness on examination, achlorhydria and a normal serum protein were the clinical findings. Radiologically, an infiltrating gastric neoplasm was suspected, and a subtotal gastrectomy was performed. The stomach was characterized by giant rugal folds and a thickened mucosa, with replacement of the secretory elements by mucus‐secreting columnar cells, and a hyperplastic proliferation of this epithelium. This case represents a hypo‐secretory variant among the various forms of hypertrophic gastropathy described.
