A tumor of the thymus gland characterized by a polymorphic infiltrate similar or identical to Hodgkin's disease has often been diagnosed as granulomatous thymoma. This report describes three such cases. Both the clinical course and the morphologic changes suggest that these patients have nodular sclerosing Hodgkin's disease in which diagnostic Reed‐Sternberg cells are present but extremely sparse. The chance of extrathoracic involvement is extremely high in comparison with all other types of thymomas. For prognostic and therapeutic purposes, these cases should be designated as “Hodgkin's disease of the thymus” and not “granulomatous” thymoma.
