A case of bronchiolar cell carcinoma in a lung with diffuse interstitial pulmonary fibrosis and atypical alveolar cell hyperplasia following long‐term busulfan therapy for chronic granulocytic leukemia is described. The literature is reviewed and the radiomimetic and mutagenic properties of busulfan are thought to cause the diffuse interstitial pulmonary fibrosis and atypical alveolar cell hyperplasia. The occurrence of bronchiolar cell carcinoma in a lung with diffuse atypical alveolar cell changes is believed to represent different phases of the same pathologic process; the probable carcinogenic role of busulfan is discussed.
