The results of 4050 needle (aspiration) biopsies, including 941 junctures of the spine, studied for 25 years have been analyzed. The cytologic study of the smears and the histopathologic interpretation of the paraffin‐embedded material were combined whenever possible. The smear technique is of great value for the study of finer cytologic details; however, it cannot be used in fixed material or in lesions with a predominance of the connective or skeletic component. With the aspirated material some histochemical techniques can be carried out, the simplicity and exactness of which permit routine application and may be helpful in differential diagnosis (alkaline phosphatase, glycogen, etc.). Aspiration biopsy which is a simple time‐saving method, without major risks, is particularly useful in deep locations such as the pelvis and the vertebral bodies.
