The clinical history and autopsy findings of a patient with a cytologically malignant thymoma which metastasized widely is reviewed. The histologic structure of the primary lesion resected at thoracotomy is of prognostic significance. This patient died 5 months following thoracotomy. A significant relationship between cytologic findings in thymic tumors and subsequent clinical behavior has been denied by some observers.
