A case report is presented of a patient with intermittent severe hypoglycemic episodes associated with a large extrapancreatic leiomyosarcoma. How the tumor produced hypoglycemia could not be defined, but the various theories are presented. The patient was able to tolerate prolonged periods of low levels of blood sugar without the development of neuropsychiatric or electroencephalographic changes. The mechanism whereby this can occur may be the breakdown of endogenous cerebral proteins, releasing metabolic energy. At an 18‐month postoperative progress examination, the patient was symptom free.
