To suppress TSH secretion, hypophysectomy was carried out in 14 patients with advanced thyroid cancer. Considered as candidate to hypophysectomy were patients with either locally advanced inoperable disease or distant metastases. Positive results were obtained in six cases: In five objective regression was observed; the last one showed an arrest of growth lasting 2 1/2 years. Five of the six responsive cases were differentiated carcinomas and one was undifferentiated. The best result was obtained in a patient with signs of cretinism, whose cancer developed in a large goiter dating from infancy.
