Adult granulosa‐cell tumor (AGCT) is an uncommon ovarian malignancy. Rarely, hepatic metastasis of this tumor may present a difficult cytologic diagnosis when such a lesion is aspirated. This case report elaborates the cytopathologic features of AGCT in the liver aspirate of a 60‐yr‐old woman. Correlation with histopathology and intraoperative touch‐imprint cytology of the subsequent hepatic resection is also presented.
