Lymphatic mapping and sentinel lymph node biopsy results in a more conservative and less morbid operation for the patient with melanoma. In addition, the ability to submit the sentinel lymph node separately for pathologic examination allows for a more detailed examination that will result in more accurate staging. The challenge for investigators now is to ascertain the clinical relevance of the “upstaging” that can occur with these new techniques. See also pages 617–27.
