To date, the presence of vascular invasion and the presence/percentage of embryonal carcinoma appear to be the most important risk factors for predicting pathologic stage in patients with clinical Stage I nonseminomatous germ cell tumors (NSGCT). This editorial addresses problems and concerns associated with the identification and clinical application of new prognosticators to guide risk‐adapted management in patients with clinical Stage I NSGCT.
