Abstract A 6‐year‐old, male, entire Jack Russell terrier was referred for noise fear and subsequently diagnosed with underlying musculoskeletal discomfort. Significant improvements in the signs were reported by the clients by initially addressing the underlying discomfort using a combination of analgesia (1.7 mg/kg carprofen once a day), environmental modification (provision of non‐slip matting) and simple management strategies (avoiding triggers and providing a safe haven). The role of discomfort in the aetiology, presentation and prognosis of noise fear cases is well recognised, but the impact of just pain management is less well documented. This case demonstrates how pain relief and simple management plans can be used in cases where pain is significantly contributing to noise fear to produce potentially meaningful improvements in behaviour. Such measures should be considered in both general practice, as well as referral centres.
