Recognizing critical circumstances for therapist-patient dyad is important to make the most of them and to avoid iatrogenia. This is shown by a clinical case where the actual circumstance of the therapist (pregnancy) is a specific trigger for the type of pathology (attachment disorder with a borderline structure). This generates a unique situation for therapeutic change, but it also implies a high risk of damage. Finally, transference and countertransference changes are analyzed, as well as technical interventions in the therapeutic process that can facilitate change and minimize risks for iatrogenic effects
