Ce patient de 65 ans consulte pour un état fébrile jusqu’à 40,2 °C avec frissons solennels, douleur pharyngée, asthénie et myalgies. Une pneumonie est suspectée et de la co-amoxicilline est prescrite. Après 3 jours, il n’y a pas d’amélioration clinique. Le patient note l’apparition d’une éruption cutanée érythémateuse avec des macules conuentes, non prurigineuse. Le rash a commencé sur le tronc, s’étendant aux extrémités et à l’abdomen, puis a disparu après 5 jours. L’antibiotique a été interrompu. Un ictère s’installe. Le patient est hospitalisé. A l’examen clinique T° 38,2 °C; TA 159/97 mm Hg; FC 110/min, FR 28/min, saturation à 93% sous 2 l d’O2. Les sclères sont jaunes, sans autres lésions muco-cutanées. Le bilan sanguin montre une anémie normochrome normocytaire à 86 g/l, une leucocytose à 27 G/l, avec 83% de neutrophiles, dont 79% segmentés, une CRP 229 mg/l et une cholestase importante avec cytolyse hépatique (ASAT et ALAT à 4 fois la norme).

Etat fébrile avec neutrophilie, éruption cutanée et perturbation des tests hépatiques
