Ancak ön tanıda şüphe olması halinde ya da mevcut patolojinin daha ayrıntılı görüntülenmesi amacıyla bilgisayarlı toraks tomografisine de başvurulabilmektedir. Çalışmamızda geriye dönük olarak çekilen bilgisayarlı toraks tomografilerin tanı koyma ve tedaviyi yönlendirme açısından sağladığı ek yararlar ve çekilen bilgisayarlı tomografilerin ne kadarının gerçekten gerekli olduğu araştırılmıştır. Yöntemler: Çocuk yoğun bakım servisimize 1 Ocak 2013 1 Ocak 2015 tarihleri arasında yatan ve bilgisayarlı toraks tomografisi çekilen tüm olgular hastane bilgi işlem sistemi kullanılarak değerlendirildi. Bilgisayarlı tomografi ve akciğer grafisi ile konulan tanıların arasında fark olup olmadığı Mc Nemar testi ile; bu iki tetkik yöntemi arasındaki uyum ise Kappa analizi ile değerlendirildi. İstatistiksel analizler için SPSS 22.0 programı kullanıldı ve p<0,05 düzeyi anlamlı kabul edildi. Bulgular: Çalışmamıza dahil edilen 54 olguda tanıları öngörme açısından akciğer grafisi ile bilgisayarlı toraks tomografisi arasında fark olmadığı (p=0,518); iki test arasında uyumun iyi derecede olduğu (Kappa: 0,632) ve bu uyumun istatistiksel olarak önemli olduğu saptandı (p<0,001). Olguların %57’sinde bilgisayarlı tomografinin tedaviyi değiştirme ve yönlendirmede etkisi saptanmadı. Ayrıca %74’ünde tanı klinik bulgular ve akciğer grafisi ile konulabildi. Sonuç: Akciğer grafisi ile pek çok hastada doğru tanı konulabilmiş ve tedavi uygun şekilde yönlendirilebilmiştir. Ayrıca istatistiksel olarak da iki tetkik arasında uyum yüksek bulunmuştur. Bu nedenlerle bilgisayarlı tomografi, özellikle çocuk hastalarda yan etkileri de göz önünde bulundurulduğunda iyi düşünülerek başvurulması gereken bir tetkiktir. Ancak yine de tanının şüpheli olduğu veya tedaviye cevap vermeyen seçili olgularda teknik üstünlükleri nedeniyle bilgisayarlı tomografinin öneminin göz ardı edilemeyeceği bir gerçektir. Anahtar Kelimeler: Bilgisayarlı toraks tomografisi, akciğer grafisi, çocuk yoğun bakım, akciğer patolojileri, endikasyon Introduction: Chest radiography is usually sufficient for the diagnosis of lung diseases in pediatric intensive care unit. However, if there is doubt about the diagnosis, thoracic computed tomography can be performed. In this study, we aimed to determine if chest computed tomography provides more information to diagnosis and treatment plan in comparison with chest radiography and discussed the necessity of thoracic computed tomography. Methods: We evaluated records of all patients who underwent chest computed tomography between 01 January 2013 and 01 January 2015 using the hospital data processing system. Mc Nemar’s test was used to investigate the presence of difference in diagnosis between computed tomography and chest X-ray. The concordance between the two imaging methods was displayed using Kappa statistics. SPSS 22.0 software was used for statistical analysis and a p value of less than 0.05 was considered statistically significant. Results: A total of 54 patients were included in the study. There was no difference between chest computed tomography and chest radiograph in predicting diagnosis (p=0.518). There was a high degree of concordance between the two methods (Kappa: 0.632) (p<0.001). Computed tomography provided no additional information or guidance for changing the treatment of thorax pathology in 57% of patients. The diagnosis could be established with clinical findings and chest X-ray in 74% of patients. Conclusion: In many patients, chest X-ray was sufficient for correct diagnosis and appropriate treatment plan. In addition, a statistically high degree of correlation was found between the two methods. For this reason, thoracic computed tomography is the diagnostic approach which should be chosen meticulously considering its side effects especially in childhood. However, importance of thoracic computed tomography is not negligible either in cases unresponsive to treatment or when the diagnosis is uncertain.

Efficiacy Comparison between Computed Tomography of the Thorax and Chest X-Rays in Diagnosing Thoracic Pathology in Pediatric Intensive Care Unit
