With the high prevalence of gastro-oesophageal reflux disease, the incidence of oesophageal adenocarcinoma is increasing in many parts of the world, particularly in the West. Therefore, recognition of Barrett's oesophagus, its premalignant stage, has become important in the management of gastro-oesophageal reflux disease. This review focuses on the evolution of knowledge since the first description of the columnar lined oesophagus, the current status and controversies surrounding Barrett's oesophagus, and problems associated with its management.  The Sri Lanka Journal of Surgery 2014; 32(4): 2-8
