The complete absence of the left pericardium is a rare condition, which is found incidentally during surgical procedures. Preoperative computerized tomography imaging is not a routine practice in open heart surgery, and the diagnosis of these cases is often overlooked. In a patient undergoing on-pump coronary artery bypass surgery, we present a rare case of a left pericardium after sternotomy.
