The aim was related knowledge levels between attitudes about health preventive behavior among State School teachers in Peru. For what we conducted an online survey in well-known State school teachers in Peru to measuring knowledge levels and their attitudes about oral health preventive behavior. We use a survey which was created for this specially study but previously checked it over by expert judges, also it was used in a pilot test α Cronbach = 0.85. We used Likert’s scale to measure knowledge and attitudes about oral health preventive behavior variables. The results was 586 state school Peruvian teachers completing this online survey. Of these, 311 (53.07%) were females, 325(55 %) in the age group of 2535 years. Knowledge level index had 47. 06% (Inadequate) and Attitudes levels index had 26.47% (adequate). Pearson Correlation Coefficient had 0.5 (low direct relationship between variables). The conclusions State school teachers surveyed had inadequate knowledge level index and inadequate attitudes levels index, showing us Intervention Plan neediness to improve oral health preventive behavior among State school teachers in this society, and does exist direct relate between variables by Pearson Correlation
