Objective: To determine the relationship between the level of perception of patient safety culture and reporting of adverse events referred to by the nursing staff a Level I hospital in the province of Ferreñafe, Lambayeque, Peru during 2015. Materials and Methods: the sample population consisted of 91 health professionals. The study was analytical, prospective, cross-sectional design and hypothesis testing was used relational descriptive. For statistical analysis the test Chisquare read 95% reliability in data collection questionnaire entitled "Questionnaire on patient safety in hospitals" of the Agency for Health Research and Quality was used was used United States (AHRQ). Results: The results showed that the level of general perception of safety culture manifested by the care team was neutral and negative (90%), it was consistently associated with a high percentage of unreported adverse events (74%). (P <0.05), with two dimensions of patient safety culture that were statistically associated with no adverse events reporting: management support for patient safety and frequency of incidents reported (p <0.05). Conclusions: support and participation of Directors is required to achieve adequate safety
