Open bite malocclusion after suffering from trauma is one type of open bite due to malposition of fragment position. This type of fracture is usually caused by a situation that the patient was unable to treat immediately because of the worst medical situation or neglected or untreated fracture and healed in malpositioned fragmen. Le Fort I osteotomy is one of surgical technique can be used to treat this type fracture and this type of fracture showed clearly that this open bite caused by Le Fort I fracture. A case of patient, male 32 year old visited the Department of Oral and Maxillofacial Surgery Dr. Hasan Sadikin Hospital with a chieft of complaint of anterior open bite, can not close his jaw and difficulty to chew properly. History of trauma one month before he appeared to Dr. Hasan Sadikin Hospital, the patient was fell down from roof of the house with down position against the floor and was immediately brought to the nearest hospital. Several examinations and treatments were perfomed and in the same day he was allowed to discharged from the hospital. Le Fort I osteotomy was performed, repositioned the upper jaw to achieve a good occlusion, and maxillae was fixed with four miniplate osteosynthesis
