ptimal control of blood pressure, lipids, and glycemia occurs in < 10% of patients with diabe- tes in national surveys, even as diabetes becomes epidemic and care for diabetes increasingly becomes the target of pay- for-performance and public reporting requirements. Complicating the task is that diabetes typically coexists with mul- tiple medical problems that compete for attention during traditional clinical vis- its; recent evidence suggests this barrier may be even more salient than the con- cept of clinical inertia. 1
