A major goal of United States health care reform is to reduce hospital readmissions. In 2012, approximately 18% of Medicare patients discharged from the hospital were readmitted within 30 days ([1][1]). According to one estimate, readmissions cost the Medicare program >$17 billion in 2004 ([2][2]).
