A 53-year-old man with ESKD underwent living donor kidney transplantation from his spouse who had a positive serum PCR for Epstein–Barr virus (EBV) at the time of donation. He received rabbit antithymocyte globulin induction and rapid corticosteroid taper with maintenance immunosuppressive therapy
