S  inus of Valsalva aneurysm (SVA) is a rare congenital heart disease. SVA complicates with rupture into a heart chamber, causing its   volume overload,   which could   lead to   heart failure and needs surgical correction.   Transthoracic echocardiography is a  n   available diagnostic tool for the diagnosis  ,   other   methods of cardiac imaging (cardiac CT and MRI)   confirm   the diagnosis and are necessary for preoperative assessment of the pathology. A 65-year-old man was admitted to hospital for   h  eart failure symptoms   which   worsened during the last couple of weeks  . Loud murmur was revealed during auscultation of the heart. Transthoracic echocardiography revealed cystic-like structure in the right ventricle adjacent to the interventricular septum. Color Doppler showed turbulent flow from the aorta to the cavity and from the cavity to the right ventricle. Diagnosis   of the right sinus   of   Valsalva   aneurysm   dissecting interventricular septum and ruptured into the right ventricle   was established. The diagnosis was confirmed by ECG-gated cardiac computed tomography.   The successful surgical correction resulted in regression of the heart chambers dilatation and dysfunction and to functional recovery of the patient
