Familial adenomatous polyposis is an autosomal dominant inherited disease inducing numerous polyps in colon and rectum and characterized by cancer formation. Unless polyps are removed at an early age, development of colon cancer from these polyps is unevitable. A-24 year old male patient admitted to the emergency service with bloody diarrhea. He lost nearly 10 kg weight in the last two months. Family history showed that his mother died due to colon cancer 15 days ago, and his 6 close relatives also died due to colon cancer. In rectal examination, polypoid mass was palpated manually. Colonoscopy demonstrated numerous polyps and rectal masses in rectum and colon, a biopsy was conducted. The pathology reveiled adenocarcinoma and the patient was refered to general surgery department. In this case report we aimed to emphasize the importance of rectal digital examination and investigation of the family history to diagnose the underlying malignity in young male patient applied to the emergency service with the complaint of bloody diarrhea. Key words: Familial adenomatous polyposis, bloody diarrhea, anemia, colon cancer
