KOMPOZİT İNLEY, İNDİREKT PORSELEN İNLEY VE CAD/CAM İNLEY SİSTEMİ İLE RESTORE EDİLEN DİŞLERİN KENAR SIZINTILARININ İNCELENMESİ: İN VİTRO ÇALIŞMA

Amac: Bu calismanin amaci; farkli yontemler ve ma- teryallerle restore edilen inley uygulamalarinin kenar sizintisi acisindan in vitro olarak degerlendirilmesidir. Gerec ve Yontem: Calismada; curuksuz ve restorasyonsuz 80 adet 3.molar insan disine sinif II (mezio-okluzal) inley kaviteler hazirlandi. Disler, her grupta 20 adet olacak sekilde dort gruba ayrildi. 1.Grup: Filtek Ultimate (3M ESPE, St. Paul, MN, ABD) kompozit, 2.Grup: Lava Ultimate CAD/CAM (3M ESPE, St. Paul, MN, ABD), 3.Grup: IPS e.max Press (Ivoclar Vivadent, Amherst, NY) seramik inleyler ve 4.Grup: IPS e.max CAD (Ivoclar Vivadent, Amherst, NY) inleyler hazirlandi. Bitirme ve polisaj islemlerinden sonra tum orneklere 5±1°C’de ve 55±1°C’de 10.000 kez termal siklus uygulandi. Tum orneklerde restorasyonun etrafindaki 1 mm’lik alan disinda kalan bolgeler iki kat tirnak cilasi ile kaplandi ve 24 saat sureyle % 0,5’lik bazik fuksin solusyonunda bekletildi. Kenar sizintisini degerlendirmek icin okluzo-gingival yonde longitudinal olarak kesildi. Boya penetrasyonu stereomikroskop altinda (30x buyutmede) incelendi ve skorlandi. Sonuclar Kruskal-Wallis, Mann-Whitney U ve Wilcoxon Signed Ranks testleri ile istatistiksel olarak degerlendirildi (Anlamlilik duzeyi p<0,05). Bulgular: Okluzal bolgede en az sizinti e.max CAD ve Lava Ultimate gruplarinda gozlendi. CAD/CAM yontemi ile hazirlanan gruplar, diger gruplara oranla okluzal bolgede anlamli derecede daha dusuk sizinti degerleri gostermistir (p=0,000). Gingival bolgedeki sizinti degerleri incelendiginde, gruplar arasinda anlamli bir fark bulunamamistir (p=0,825). Sonuc: Mikrosizinti degerleri kiyaslandiginda CAD/ CAM ile uretilen inley gruplari ozellikle okluzal bolgede daha basarili sonuclar verirken, servikal bolgede grup- lar arasinda anlamli bir farklilik bulunmadi. Bununla beraber, in vitro calismalarin in vivo calismalarla desteklenmesine ihtiyac vardir. Anahtar Sozcukler: CAD/CAM sistemi, direkt kompozit inley, indirekt porselen inley, mikrosizinti MARGINAL LEAKAGE EVALUATION OF COMPOSITE INLAY, INDIRECT PORCELAIN INLAY AND TEETH RESTORED WITH CAD/CAM SYSTEMS: IN VITRO STUDY ABSTRACT Aim: The aim of this study was to evaluate the in vitro marginal leakage of inlays performed with different methods and materials. Materials and Methods: In this study, class II mesio-occlusal cavities were prepared on 80 caries and restoration free human third molar teeth. The teeth were divided into four groups and 20 teeth each. 1. Group: Filtek Ultimate (3M ESPE, St. Paul, MN, ABD) direct composite inlays, 2.Group: Lava Ultimate CAD/CAM (3M ESPE, St. Paul, MN, ABD) inlays, 3.Group: IPS e.max Press (Ivoclar Vivadent, Amherst, NY) ceramic inlays and 4.Group: IPS e.max CAD (Ivoclar Vivadent, Amherst, NY) inlays were prepared. After finishing and polishing procedures 10.000 thermal cycles between 5±1°C and 55±1°C were applied to all specimens. All samples, except 1 mm around the restoration margin, were coated with two layers of nail varnish, and were suspended in 0.5 % basic fuchsine solution for 24 hours. To evaluate the microleakage, the teeth were cut longitudinally in occluso-gingival direction. The dye penetration was examined under a stereomicroscope (30x magni- fication) and scored. The results were statistically analyzed by using Kruskal-Wallis, Mann-Whitney U and Wilcoxon Signed Ranks tests (Significance level of p<0.05). Results: In occlusal part, lowest microleakage values were obtained in e.max CAD and Lava groups. CAD/CAM groups showed statistically significant lower microleakage values than the other groups in occlusal part (p=0,000). In gingival part, there were no significant difference between the groups (p=0,825). Conclusion: When microleakage values of occlusal regions were compared, CAD / CAM generated inlay groups gave better results; but in cervical regions, no significant difference was founded between the groups. However, there is a need to support the in vitro findings by in vivo studies. Key Words: CAD/CAM system, direct composite inlay, indirect porcelain inlay, microleakage .
